New Patients

If you live in our practice area and wish to register as a patient at our surgery then we need certain information from you and each member of your family so as to be able to trace your previous medical records. Before requesting registration, please confirm that you fall in our catchment area by checking here .

Before you register, it is helpful to know your NHS number. You can use the NHS website to find your NHS number.

Medical treatment is available from the date of registration, however all new patients are required to undergo a New Patient Check with a health care assistant within three months of registering. Please contact reception for further information.

NAMED GP: We assign all new and existing patients with a named accountable GP to oversee their care.

For patients aged 75 and over, the named accountable GP is responsible for:

  • working with health and social care professionals to deliver care that meets the needs of the patient.
  • ensuring that these patients have access to a health check.

Patients can still choose to see any PG in the surgery. We will make reasonable efforts to accomodate their preference.
